Freedom of Information Requests
Under the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) and the Personal Health Information Protection Act (PHIPA), you can submit a request to the Toronto Parking Authority to:
- Request information
- Request your personal information or personal health information
- Correct your personal information or personal health information
Make a request
Complete the request form
Provide a detailed description of the information or records you are requesting. Where applicable, include the area or department that may have the information and the dates of the information you are requesting.
Include supporting documents
If you are requesting personal information or personal health information, include a photocopy of signed, government-issued identification, such as a driver’s licence or passport.
If you are requesting a correction to personal information or personal health information, indicate the correction requested and include supporting documentation.
Pay the application fee
Include the $5 application fee, payable by cash, cheque or money order to the Toronto Parking Authority. The application fee is mandatory and is not subject to waiver.

Mail your completed form, payment and any required supporting documents to:

Processing time
The time for processing access requests is 45 days. A time extension, however, may be applied where necessary.
